冠心病患者 PCI 后运动恐惧的潜在剖面分析 及其影响因素研究

摘要:

 目的 基于潜在剖面分析探讨冠心病患者经皮冠状动脉介入治疗(PCI)后运动恐惧的类别,探讨不 同类别运动恐惧的影响因素。方法 采用便利抽样法选取2022年11月—2023年4月西安交通大学第一附属医院心血管 内科收治的冠心病患者为调查对象。使用一般资料调查表、心脏病患者运动恐惧量表(TSK-Heart)、冠心病PCI术后 患者赋权护理评价量表进行调查。基于潜在剖面分析探讨冠心病患者PCI后运动恐惧的类别,冠心病患者PCI后不同类 别运动恐惧的影响因素分析采用多分类Logistic回归分析。结果 共发放问卷241份,回收有效问卷236份,有效回收率 为97.9%。潜在剖面分析结果显示,冠心病患者PCI后运动恐惧可分为3个潜在类别:低度运动恐惧79例(33.5%),中 度运动恐惧138例(58.5%),高度运动恐惧19例(8.0%)。低度运动恐惧、中度运动恐惧、高度运动恐惧者婚姻状 况、文化程度、子女数量、医疗保险类型、冠心病家族史、纽约心脏病协会(NYHA)分级、PCI穿刺部位、冠心病 PCI术后患者赋权护理评价量表评分比较,差异有统计学意义(P<0.05)。多分类Logistic回归分析结果显示,文化程 度、冠心病PCI术后患者赋权护理评价量表评分是冠心病患者PCI后中度运动恐惧的独立影响因素,婚姻状况、PCI穿 刺部位、冠心病PCI术后患者赋权护理评价量表评分是冠心病患者PCI后高度运动恐惧的独立影响因素(P<0.05)。 结论 冠心病患者PCI后运动恐惧存在不同类别,文化程度、冠心病PCI术后患者赋权护理评价量表评分是冠心病患者 PCI后中度运动恐惧的独立影响因素,婚姻状况、PCI穿刺部位、冠心病PCI术后患者赋权护理评价量表评分是冠心病 患者PCI后高度运动恐惧的独立影响因素。

Abstract:

Objective To analyze the categories of kinesophobia in patients with coronary heart disease after percutaneous coronary intervention (PCI) based on potential profile analysis, and explore the influencing factors of different categories of kinesophobia. Methods Coronary heart disease patients admitted to Department of Cardiovascular Medicine of the First Affiliated Hospital of Xi'an Jiaotong University from November 2022 to April 2023 were selected as the survey subjects using convenience sampling method. The General Information Questionnaire, Tampa Scale for Kinesiophobia Heart (TSK-Heart) , and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI were used to conduct an investigation. The categories of kinesophobia in patients with coronary heart disease after PCI were explored based on potential profile analysis, and multinomial Logistic regression analysis was used to explore the influencing factors of different categories of kinesophobia in patients with coronary heart disease after PCI. Results A total of 241 questionnaires were distributed, and 236 valid questionnaires were collected, with an effective response rate of 97.9%. The results of the potential showed that kinesophobia in patients with coronary heart disease after PCI could be categorized into 3 potential categories: 79 cases (33.5%) in the low kinesophobia, 138 cases (58.5%) in the moderate kinesophobia, and 19 cases (8.0%) in the high kinesophobia. There was statistically significant differences in marital status, education level, number of children, type of health insurance, family history of coronary heart disease, New York Heart Association (NYHA) classification, PCI puncture site, and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI score in the patients with low kinesophobia, moderate kinesophobia, and high kinesophobia (P < 0.05) . Multinomial Logistic regression analysis showed that education level and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI score were the independent influencing factors of moderate kinesophobia in patients with coronary heart disease after PCI, and marital status, PCI puncture site, and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI score were the independent influencing factors of high kinesophobia in patients with coronary heart disease after PCI (P < 0.05) . Conclusion There are different categories of kinesophobia in patients with coronary heart disease after PCI, education level and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI score are the independent influencing factors of moderate kinesophobia in patients with coronary heart disease after PCI, and marital status, PCI puncture site, and Empowerment Nursing Care Evaluation Scale for Coronary Heart Disease Patients after PCI score are the independent influencing factors of high kinesophobia in patients with coronary heart disease after PCI. profile analysis
