摘要:

目的 采用Meta分析方法评价虚拟现实(VR)技术在心脏康复患者中的应用效果。方法 计算机检索中国知网、中国生物医学文献数据库、万方数据知识服务平台、维普网、Web of Science、PubMed、Embase、Cochrane Library公开发表的关于VR技术在心脏康复患者中应用效果的文献,其中试验组在常规心脏康复的基础上进行VR技术干预,对照组进行常规心脏康复。检索时限为建库至2023年5月。由2名研究者按照文献纳入与排除标准独立完成文献筛选并提取资料,同时进行文献质量评价。采用RevMan 5.4进行Meta分析,比较两组6 min步行距离、焦虑自评量表(SAS)评分、抑郁自评量表 (SDS)评分、压力感知问卷(PSQ)评分、出勤率。结果 最终纳入Meta分析的文献10篇,共涉及1 021例患者。文献质量评价结果显示,1篇为A级,9篇为B级。Meta分析结果显示,试验组6 min步行距离长于对照组〔均数差(MD)=29.22,95%CI(22.94,35.51)〕,SAS评分〔标准化均数差(SMD)=-1.90,95%CI(-3.35,-0.45)〕、SDS评分〔SMD=-1.44,95%CI(-2.56,-0.31)〕、PSQ评分〔MD=-8.74,95%CI(-13.04,-4.43)〕低于对照组(P<0.05);试验组与对照组出勤率比较,差异无统计学意义〔相对危险度(RR)=0.97,95%CI(0.81,1.16),P>0.05〕。结论 VR技术能够提高心脏康复患者的运动耐力,减轻焦虑、抑郁情绪,降低压力水平,但其在改善患者治疗依从性方面并无明显优势。

Abstract:

Objective To evaluate the application effect of virtual reality (VR) technology in cardiac rehabilitationpatients by meta-analysis. Methods The literature on the application effect of VR technology in cardiac rehabilitation patientspublished on the CNKI, China Biomedical Literature Database, Wanfang Database, VIP, Web of Science, PubMed, Embase, andCochrane Library were searched by computer. The experimental group received VR technology intervention on the basis of routinecardiac rehabilitation, while the control group received routine cardiac rehabilitation. The search deadline was from the databaseestablishment date to May 2023. According to the inclusion and exclusion criteria, two researchers independently completedliterature screening and data extraction, and carried out literature quality evaluation. Meta-analysis was performed using RevMan5.4. The 6 min walking distance, Self-rating Anxiety Scale (SAS) score, Self-rating Depression Scale (SDS) score, Perception ofStress Questionnaire (PSQ) score and attendance rate were compared between the two groups. Results A total of 10 literaturewere included in meta-analysis, including 1 021 patients. The results of literature quality evaluation showed that 1 paper wasgrade A and 9 papers were grade B. The results of meta-analysis showed that the 6 min walking distance of the experimental groupwas longer than that of the control group [mean difference (MD) =29.22, 95%CI (22.94, 35.51) ] , and the SAS score [standardizedmaen difference (SMD) =-1.90, 95%CI (-3.35, -0.45) ] , SDS score [SMD=-1.44, 95%CI (-2.56, -0.31) ] and PSQ score [MD=-8.74, 95%CI (-13.04, -4.43) ] were lower than those of the control group (P < 0.05) . There was no significant difference inattendance rate between the experimental group and the control group [relative risk (RR) =0.97, 95%CI (0.81, 1.16) , P > 0.05] . Conclusion VR technology can improve exercise endurance and reduce anxiety, depression and stress levels in cardiacrehabilitation patients, but it has no obvious advantages in improving patients' treatment compliance.
