基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式构建

摘要:

目的 构建基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式。方法 成立研究小组,通过现况调查、文献检索制定基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式初稿;采用德尔菲法进行专家函询,制定基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式。采用专家函询表有效回收率、提出修改意见专家占比评估专家积极程度;采用专家权威系数评估专家权威程度;采用肯德尔和谐系数、变异系数(CV)评估专家意见协调程度;采用重要性评分评估专家意见集中程度。结合层次分析法,计算各指标的原始权重、组合权重及一致性系数。结果 第1轮专家函询的有效回收率为87.5%,提出修改意见专家占比为61.9%(13/21);第2轮专家函询的有效回收率为100.0%,提出修改意见专家占比为0。第1轮专家函询的专家权威系数为0.874,第2轮专家函询的专家权威系数为0.879。第1轮专家函询的总体CV为0.087,肯德尔和谐系数为0.221;第2轮专家函询的总体CV为0.016,肯德尔和谐系数为0.378。第1轮专家函询显示,各指标的重要性评分为3.43~5.00分;第2轮专家函询显示,各指标的重要性评分为4.52~5.00分。第1轮专家函询共有13名专家提出修改建议,经研究小组讨论后确定删除3个指标、修改24个指标、合并2个指标;第2轮专家函询无专家提出修改意见。最终形成了基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式,其包括5个一级指标、15个二级指标、53个三级指标;各指标的重要性评分为4.52~5.00分,CV为0~0.113,满分率为52.4%~100.0%,原始权重为0.077~1.000,组合权重为0.003~0.041,一致性系数均<0.1。结论 本研究构建的基于佩普劳人际关系理论的老年2型糖尿病合并高血压患者健康管理模式共包括5个一级指标、15个二级指标和53个三级指标,其具有较好的科学性和可靠性,可为临床管理提供参考。

Abstract:

Objective To construct a health management model for elderly patients with type 2 diabetes andhypertension based on Peplau's interpersonal relationship theory. Methods A research group was set up to formulate the firstdraft of health management model for elderly patients with type 2 diabetes and hypertension based on Peplau's interpersonalrelationship theory through current investigation and literature search. Delphi method was used for expert correspondence toformulate the health management model for elderly patients with type 2 diabetes and hypertension based on Peplau's interpersonalrelationship theory. The effective recovery rate of expert letter consultation form and the proportion of experts who put forwardmodification suggestions were used to evaluate the degree of expert activeness. The expert authority coefficient was used toevaluate the degree of expert authority. Kendall coefficient of harmony and coefficient of variation (CV) were used to evaluate thedegree of expert opinion coordination. The importance score was used to evaluate the concentration of expert opinions. Combined with analytic hierarchy process, the original weight, combined weight and consistency coefficient of each index were calculated.Results The effective recovery rate of the first round of expert correspondence was 87.5%, and 61.9% (13/21) of the expertsproposed amendments; the effective recovery rate of the second round of expert correspondence was 100.0%, and the proportionof experts put forward modification opinions was 0. The expert authority coefficient of the first round of expert correspondence was0.874, and the expert authority coefficient of the second round of expert correspondence was 0.879. The overall CV of the firstround of expert correspondence was 0.087, and the Kendall concordance coefficient was 0.221; the overall CV of the second roundof expert correspondence was 0.016, and the Kendall harmony coefficient was 0.378. The first round of expert correspondenceshowed that the importance score of each indicator was 3.43-5.00; the second round of expert correspondence showed that theimportance score of each indicator was 4.52-5.00. In the first round of expert consultation, 13 experts put forward suggestions formodification. After discussion, the research group determined to delete 3 indicators, modify 24 indicators, and merge 2 indicators.In the second round of expert consultation, no experts put forward modification suggestions. Finally, the health managementmodel for elderly patients with type 2 diabetes and hypertension based on Peplau's theory was formed, it included 5 first levelindicators, 15 second level indicators, and 53 third level indicators. The importance score of each index was 4.52-5.00, CV was0-0.113, the full score rate was 52.4%-100.0%, the original weight was 0.077-1.000, the combined weight was 0.003-0.041,and the consistency coefficient was < 0.1. Conclusion The health management model for elderly patients with type 2 diabetesand hypertension based on Peplau's interpersonal relationship theory established in this study includes 5 first level indicators,15 second level indicators and 53 third level indicators, which is scientific and reliable, and can provide reference for clinicalmanagement.
