摘要:

目的 探讨胰岛素样生长因子1受体(IGF-1R)在结直肠癌合并2型糖尿病患者中的表达情况及其与临床特征的关系。方法 回顾性收集2014年3月至2017年1月石家庄市人民医院收治的结直肠癌患者59例,根据患者是否合并2型糖尿病将其分为合并组29例与对照组30例。收集患者的性别、年龄、肿瘤最大直径、临床分期、肿瘤分化程度、淋巴结转移情况,采用免疫组化法检测结直肠癌组织中IGF-1R表达情况。术后随访36个月,随访截至2020-01-30,记录患者生存情况。比较两组临床资料及结直肠癌组织中IGF-1R表达情况,不同临床特征患者结直肠癌组织中IGF-1R表达阳性率。结果 两组肿瘤最大直径、肿瘤分化程度、结直肠癌组织中IGF-1R表达情况比较,差异有统计学意义(P<0.05)。不同年龄、临床分期、肿瘤分化程度、淋巴结转移情况患者结直肠癌组织中IGF-1R表达阳性率比较,差异无统计学意义(P>0.05);不同性别、肿瘤最大直径患者结直肠癌组织中IGF-1R表达阳性率比较,差异有统计学意义(P<0.05)。对照组不同性别、年龄、肿瘤最大直径、临床分期、肿瘤分化程度、淋巴结转移情况患者结直肠癌组织中IGF-1R表达阳性率比较,差异无统计学意义(P>0.05)。合并组不同年龄、临床分期、肿瘤分化程度、淋巴结转移情况患者结直肠癌组织中IGF-1R表达阳性率比较,差异无统计学意义(P>0.05);合并组不同性别、肿瘤最大直径患者结直肠癌组织中IGF-1R表达阳性率比较,差异有统计学意义(P<0.05)。合并组死亡率高于对照组(P<0.05)。结论 结直肠癌合并2型糖尿病患者IGF-1R表达阳性率升高,且其与性别及肿瘤最大直径有关,结直肠癌合并2型糖尿病患者死亡率升高。

Abstract:

Objective To investigate the expression of insulin-like growth factor 1 receptor (IGF-1R) in patientswith colorectal cancer (CRC) complicated with type 2 diabetes mellitus (T2DM) and its relationship with clinical characteristics.Methods A total of 59 patients with CRC admitted in the Department of Pathology of Shijiazhuang People's Hospital from March2014 to January 2017 were retrospectively collected. The patients were divided into combined group (n=29) and control group(n=30) according to whether they had T2DM. The gender, age, maximum diameter of tumor, clinical stage, degree of differentiationand lymph node metastasis of the patients were collected, and the expression of IGF-1R in colorectal cancer tissues was detectedby immunohistochemistry. The patients were followed up for 36 months after surgery, and the follow-up time up to January 30,2020, and the survival condition of the patients was recorded. The clinical data and IGF-1R expression in colorectal cancertissues were compared between the two groups, the IGF-1R expression positive rate in colorectal cancer tissues were comparedin patients with different clinical characteristics. Results There was statistically significant difference in maximum diameterof tumor, degree of differentiation and IGF-1R expression in colorectal cancer tissues between the two groups (P < 0.05) . There was no statistically significant difference in IGF-1R expression positive rate in colorectal cancer tissues in patients with differentage, clinical stage, degree of differentiation and lymph node metastasis (P > 0.05) . There was statistically significant difference inIGF-1R expression positive rate in colorectal cancer tissues in patients with different gender and maximum diameter of tumor (P <0.05) . There was not statistically significant difference in IGF-1R expression positive rate in colorectal cancer tissues in patientswith different gender, age, maximum diameter of tumor, clinical stage, degree of differentiation and lymph node metastasis in thecontrol group (P > 0.05) . There was no statistically significant difference in IGF-1R expression positive rate in colorectal cancertissues in patients with different age, clinical stage, degree of differentiation and lymph node metastasis in the combined group (P> 0.05) . There was statistically significant difference in IGF-1R positive expression rate in colorectal cancer tissues in patientswith different gender and maximum diameter of tumor in the combined group (P < 0.05) . The mortality rate in the combined groupwas higher than that in the control group (P < 0.05) . Conclusion The IGF-1R expression positive rate in CRC complicatedwith T2DM patients is increased, and it is related to gender and tumor maximum diameter. The mortality rate of patients with CRCcomplicated with T2DM is increased.
