摘要:

目的 分析急性缺血性卒中(AIS)患者半乳糖凝集素3(Gal-3)水平及其与卒中后认知障碍(PSCI)的关系。方法 选取2021年10月至2022年5月在石河子大学医学院第一附属医院神经内科就诊的AIS患者98例为研究对象。收集患者一般资料、Gal-3水平、蒙特利尔认知评估量表(MoCA)评分。AIS患者Gal-3水平与MoCA评分的相关性分析采用Pearson相关分析;采用多因素Logistic回归分析探讨AIS患者发生PSCI的影响因素;采用ROC曲线分析Gal-3水平对AIS患者发生PSCI的预测价值。结果 Pearson相关分析结果显示,AIS患者Gal-3水平与MoCA评分及注意力、延迟记忆、定向能力评分呈负相关(P<0.05)。多因素Logistic回归分析结果显示,年龄、Gal-3水平是AIS患者发生PSCI的影响因素(P<0.05)。ROC曲线分析结果显示,Gal-3水平预测AIS患者发生PSCI的曲线下面积为0.661[95%CI(0.553,0.770)],最佳截断值为2 367.5μg/L,灵敏度为81.4%,特异度为54.5%。结论 AIS患者Gal-3水平升高,其与患者认知功能下降有关,是AIS患者发生PSCI的危险因素,但Gal-3水平对AIS患者发生PSCI有较低预测价值。

Abstract:

 Objective To analyze the level of galectin-3 (Gal-3) and its relationship with post-stroke cognitiveimpairment (PSCI) in patients with acute ischemic stroke (AIS) . Methods A total of 98 AIS patients treated in the Department ofNeurology of First Affiliated Hospital, Shihezi University School of Medicine from October 2021 to May 2022 were selected as theresearch objects. General information, Gal-3 level and Montreal Cognitive Assessment (MoCA) scores of patients were collected.The correlation between Gal-3 level and MoCA score in AIS patients was analyzed by Pearson correlation analysis. MultivariateLogistic regression analysis was used to explore the influencing factors of PSCI in AIS patients. ROC curve was used to analyzethe value of Gal-3 level in predicting PSCI in AIS patients. Results Pearson correlation analysis showed that the level of Gal3 was negatively correlated with the total score of MoCA and the scores of attention, delayed memory and orientation ability inAIS patients (P < 0.05) . Multivariate Logistic regression analysis showed that age and Gal-3 level were the influencing factorsof PSCI in AIS patients (P < 0.05) . ROC curve analysis showed that the area under the curve of Gal-3 level in predicting PSCIin AIS patients was 0.661 [95%CI (0.553, 0.770) ] , the best cut-off value was 2 367.5 μg/L, the sensitivity was 81.4%, and thespecificity was 54.5%. Conclusion The increased level of Gal-3 in AIS patients is associated with cognitive decline, which is arisk factor for PSCI in AIS patients, while the level of Gal-3 has lower predictive value for PSCI in AIS patients.
