摘要：

目的 探讨沙库巴曲缬沙坦联合认知行为疗法(CBT)治疗慢性心力衰竭(CHF)的效果。方法 选取2020年12月至2021年5月秦皇岛军工医院心内一科收治的CHF患者40例为对照组,2021年6—12月该医院收治的CHF患者40例为观察组。对照组在常规治疗的基础上接受沙库巴曲缬沙坦治疗,观察组在对照组的基础上接受CBT。比较两组治疗前和治疗8周后汉密尔顿焦虑量表(HAMA)评分、明尼苏达心功能不全生命质量量表(MLHFQ)评分以及心功能指标[N末端脑钠肽前体(NT-proBNP)、6 min步行距离、左心室射血分数(LVEF)]。结果 治疗8周后两组HAMA、MLHFQ评分分别低于本组治疗前,且观察组低于对照组(P<0.05)。治疗8周后两组NT-proBNP分别低于本组治疗前,6 min步行距离分别长于本组治疗前,LVEF分别高于本组治疗前,且观察组NT-proBNP低于对照组, 6 min步行距离长于对照组,LVEF高于对照组(P<0.05)。结论 沙库巴曲缬沙坦联合CBT能有效缓解CHF患者焦虑情绪,提高其生活质量,减轻心肌损伤,改善心功能。

Abstract：

【Abstract】　Objective To explore the effect of sacubitril/valsartan combined with cognitive behavioral therapy(CBT) in the treatment of chronic heart failure (CHF) . Methods Fourty CHF patients admitted to the Department of Cardiologyof Qinhuangdao Jungong Hospital from December 2020 to May 2021 were selected as the control group, and 40 CHF patientsadmitted to the same hospital from June to December 2021 were selected as the observation group. The control group receivedsacubitril/valsartan based on the conventional treatment, while the observation group received CBT on the basis of the controlgroup. The Hamilton Anxiety Scale (HAMA) score, Minnesota Living with Heart Failure Questionnaire (MLHFQ) score andcardiac function indicators [N-terminal pro brain natriuretic peptide (NT-proBNP) , 6-min walk distance, left ventricular ejectionfraction (LVEF) ] before treatment and at 8 week after treatment were compared between the two groups. Results After 8 weeksof treatment, the HAMA and MLHFQ scores of the two groups were lower than those before treatment, respectively, and the HAMAand MLHFQ scores of the observation group were lower than those of the control group (P < 0.05) . After 8 weeks of treatment,the NT-proBNP of the two groups was lower than that before treatment, the 6-min walking distance was longer than that beforetreatment, and LVEF was higher than that before treatment, respectively, and the NT-proBNP of the observation group was lowerthan that of the control group, the 6-min walking distance was longer than that of the control group, LVEF was higher than thatof the control group (P < 0.05) . Conclusion Sacubitril/valsartan combined with CBT can effectively relieve anxiety of CHFpatients, improve their quality of life, reduce their myocardial damage, and improve their cardiac function.
