参草通脉复方对慢性心力衰竭模型大鼠心肌重构及心肾功能的影响研究

摘要:

目的 分析参草通脉复方对慢性心力衰竭(CHF)模型大鼠心肌重构及心肾功能的影响。方法 2019年9月至2020年2月,将50只SPF级12周龄体质量为300~350 g的雄性大鼠采用随机数字表法分为正常组、假手术组、模型组、中药组、西药组,各10只。正常组大鼠不进行手术,假手术组大鼠进行手术但不建立CHF模型,模型组、中药组、西药组大鼠建立CHF模型;建模成功后,中药组大鼠给予参草通脉复方,西药组大鼠给予福辛普利和蒸馏水,正常组、假手术组、模型组大鼠给予蒸馏水灌胃,共干预28 d。比较各组大鼠心肌组织基质金属蛋白酶(MMP)-9、血管紧张素Ⅱ-1型受体(AT1R)表达水平及干预前后内生肌酐清除率(Ccr)、B型利钠肽(BNP)水平。结果 干预后假手术组、模型组大鼠心肌组织MMP-9、AT1R表达水平高于正常组,西药组大鼠心肌组织AT1R表达水平高于正常组(P<0.05);模型组大鼠心肌组织MMP-9、AT1R表达水平高于假手术组、中药组、西药组(P<0.05);中药组大鼠心肌组织AT1R表达水平低于假手术组(P<0.05)。假手术组、模型组、中药组、西药组大鼠干预前Ccr、BNP水平高于正常组(P<0.05);假手术组大鼠干预后BNP水平高于正常组(P<0.05);模型组、中药组、西药组大鼠干预后Ccr、BNP水平高于正常组、假手术组(P<0.05);中药组、西药组大鼠干预后Ccr、BNP水平低于模型组(P<0.05);西药组大鼠干预后Ccr、BNP水平高于中药组(P<0.05)。假手术组、中药组、西药组大鼠干预后Ccr、BNP水平分别低于本组干预前(P<0.05);模型组大鼠干预后BNP水平高于本组干预前(P<0.05)。结论 参草通脉复方能够通过降低CHF模型大鼠心肌组织MMP-9、AT1R表达水平来抑制其心肌重构进程,且与福辛普利相比,参草通脉复方可更有效地改善大鼠的心肾功能。

Abstract:

【Abstract】 Objective To analyze the effect of Shencao Tongmai compound on myocardial remodeling and cardiorenalfunction in rats with chronic heart failure (CHF) . Methods From September 2019 to February 2020, 50 SPF 12-week-old malerats with a body weight of 300-350 g were randomly divided into normal group, sham group, model group, traditional Chinesemedicine group, western medicine group, with 10 rats in each group. The rats in normal group did not undergo operation, therats in sham group underwent operation but did not establish the CHF model, and the rats in model group, traditional Chinesemedicine group and western medicine group established the CHF model. After the model was successfully established, rats inthe traditional Chinese medicine group were given Shencao Tongmai compound, rats in the western medicine group were givenfosinopril and distilled water, and rats in normal group, sham group and model group were given distilled water for gavage, for atotal of 28 days of intervention. The expression levels of matrix metalloproteinase (MMP) -9, angiotensin Ⅱ type 1 receptor (AT1R)and the levels of creatinine clearance (Ccr) and brain natriuretic peptide (BNP) before and after intervention in each group werecompared. Results After intervention, MMP-9 and AT1R expression levels in the myocardial tissue of rats in sham group andmodel group were higher than those in normal group, and AT1R expression level in the myocardial tissue of the rats in westernmedicine group was higher than that in normal group (P < 0.05) . MMP-9 and AT1R expression levels in the myocardial tissueof rats in model group were higher than those in sham group, traditional Chinese medicine group and western medicine group (P< 0.05) . AT1R expression level in the myocardial tissue of the rats in traditional Chinese medicine group was lower than thatin sham group (P < 0.05) . Ccr and BNP levels before intervention of the rats in sham group, model group, traditional Chinesemedicine group and western medicine group were higher than those in normal group (P < 0.05) . BNP level after intervention of therats in sham group was higher than that in normal group (P < 0.05) . Ccr and BNP levels after intervention of the rats in model group,traditional Chinese medicine group and western medicine group were higher than those in normal group and sham group (P < 0.05) .Ccr and BNP levels after intervention of rats in traditional Chinese medicine group and western medicine group were lower thanthose in model group (P < 0.05) . Ccr and BNP levels after intervention of rats in western medicine group were higher than those intraditional Chinese medicine group (P < 0.05) . In sham group, traditional Chinese medicine group and western medicine group, Ccrand BNP levels after intervention were lower than those before intervention, respectively (P < 0.05) . In model group, BNP level ofrats after intervention was higher than that before intervention (P < 0.05) . Conclusion Shencao Tongmai compound can inhibit theprocess of myocardial remodeling by reducing the expression levels of MMP-9 and AT1R in the myocardial tissue of rats with CHF.And compared with fosinopril, Shencao Tongmai compound can more effectively improve the cardiorenal function of rats.
