冠心病患者血清长链非编码RNAH19和微小RNA-22表达水平及其临床意义研究

摘要:

背景冠心病(CHD)是常见的心血管疾病,其发病过程是由多基因调控、多细胞参与的一种复杂慢性炎症过程。长链非编码RNA(lncRNA)和微小RNA(miRNA)在CHD患者中表达异常,并在其发生发展中发挥重要作用。目的 探究CHD患者血清lncRNA H19、miRNA-22表达水平及其临床意义。方法 选取2019年6月至2021年6月在陕西省人民医院就诊的CHD患者113例作为试验组,并选择同期于本院体检的健康志愿者88例作为对照组。采用RTPCR法检测两组受试者血清lncRNA H19、miR-22表达水平;采用ELISA检测血清炎性因子〔白介素6(IL-6)、肿瘤坏死因子α(TNF-α)、超敏C反应蛋白(hs-CRP)〕、内皮功能指标〔血管内皮生长因子(VEGF)、内皮素1(ET-1)〕水平;采用ROC曲线评估血清lncRNA H19、miR-22表达水平及其联合对CHD的诊断价值;并采用Pearson相关分析探讨CHD患者血清lncRNA H19、miR-22表达水平与血清炎性因子、内皮功能指标水平的相关性。结果 试验组患者血清lncRNA H19、miR-22表达水平高于对照组(P<0.05)。试验组患者血清IL-6、TNF-α、hs-CRP、VEGF、ET-1水平高于对照组(P<0.05)。血清lnc RNA H19、mi R-22表达水平及其联合诊断CHD的AUC分别为0.827、0.817和0.894。血清lncRNA H19、miR-22表达水平与IL-6、TNF-α、hs-CRP、ET-1、VEGF水平均呈正相关(P<0.05)。结论CHD患者血清lncRNA H19、miR-22表达水平升高,其对CHD有一定诊断价值,且二者联合的诊断价值更大;此外,其与血清炎性因子、内皮功能指标水平均呈正相关。

Abstract:

【Abstract】 Background Coronary heart disease (CHD) is a common cardiovascular disease. Its pathogenesisis a complex chronic inflammatory process by polygene regulation and multicellular involvement. Long non-coding RNA(lncRNA) and microRNA (miRNA) are abnormally expressed in patients with CHD and play an important role in itsoccurrence and development. Objective To investigate the serum expression level and clinical significance of lncRNAH19 and miR-22 in patients with CHD.MethodsA total of 113 CHD patients admitted to Shaanxi Provincial People'sHospital from June 2019 to June 2021 were selected as the experimental group, and 88 healthy volunteers who underwent physicalexamination in the same hospital during the same period were selected as the control group. The expression level of lncRNAH19 and miR-22 in serum of the two groups were detected by RT-PCR. The levels of inflammatory factors [interleukin 6 (IL-6) , tumor necrosis factor α(TNF-α) , hypersensitive C-reactive protein (hs-CRP) ] , endothelial function indexes [vascularendothelial growth factor (VEGF) and endothelin-1 (ET-1) ] were determined by ELISA. ROC curve was used to evaluate thediagnostic value of serum expression level of lncRNA H19 miR-22 and their combination for CHD. Pearson correlation analysiswas used to investigate the correlation between serum expression level of lncRNA H19 and miR-22 and levels of inflammatoryfactors and endothelial function indexes. Results The serum expression level of lncRNA H19 and miR-22 in the experimentalgroup was higher than that in the control group (P < 0.05) . The serum levels of IL-6, TNF-α, hs-CRP, VEGF and ET-1 in theexperimental group were higher than those in the control group (P < 0.05) . The AUC of serum expression level of lncRNA H19,miR-22 and their combination for the diagnosis of CHD were 0.827, 0.817 and 0.894, respectively. Serum expression level oflncRNA H19 and miR-22 was significantly positively correlated with level of IL-6, TNF-α, hs-CRP, ET-1 and VEGF (P <0.05) .Conclusion Serum expression level of lncRNA H19 and miR-22 are highly expressed in patients with CHD. They havecertain value for the diagnosis of CHD, and their combination have higher value for the diagnosis of CHD. Furthermore, they arepositively correlated with the levels of serum inflammatory factors and endothelial function indexes.
