摘要:
背景氧自由基可能与血管性痴呆的发生有关,而尿酸是人类血液中含量最丰富的天然抗氧化剂。近年来有不少关于中国人尿酸水平与血管性痴呆关系的研究,但其结论并不一致。目的采用Meta分析方法综合评价中国人尿酸水平与血管性痴呆的关系。方法计算机检索PubMed、Web of Science、Cochrane Library、中国知网、中国生物医学文献数据库、维普网和万方数据知识服务平台,检索时限为从各数据库建库至2021年7月。采用主题词和自由词相结合的方式检索中国人尿酸水平与血管性痴呆关系的病例对照研究。病例组为血管性痴呆患者,对照组为非血管性痴呆患者。由2名研究者独立提取资料并评价纳入文献的方法学质量,意见不一致时由第3名研究者裁定。采用RevMan 5.2软件进行Meta分析。采用Stata 11.0统计学软件进行敏感性分析和发表偏倚评估。结果最终纳入文献12篇,共包含患者1 530例,其中病例组890例,对照组640例。纳入的12篇文献中,有2篇文献患者的平均年龄≤65岁,10篇文献患者的平均年龄> 65岁。方法学质量评价结果显示,纳入文献的纽卡斯尔-渥太华量表(NOS)评分均≥6分,属于高质量文献。Meta分析结果显示,病例组与对照组尿酸水平比较,差异无统计学意义[MD=10.66,95%CI(-23.44,44.76),P=0.54]。亚组分析结果显示,在年龄≤65岁患者中,病例组尿酸水平高于对照组[MD=144.60,95%CI(16.71,272.49),P=0.03]。敏感性分析结果显示,剔除每篇文献前后的合并效应量均无明显变化,提示本研究所得结果是稳定的。Begg检验结果显示,P=0.602;Egger检验结果显示,P=0.476;提示纳入文献无明显发表偏倚。结论中国人尿酸水平与血管性痴呆的发生可能无关,但在≤65岁人群中,尿酸水平与血管性痴呆的发生可能有关。
Abstract:
【Abstract】 Background Oxygen free radicals may be related to vascular dementia, and uric acid is the mostabundant natural antioxidant in human blood. In recent years, many researchers have studied the relationship between uric acidand vascular dementia in the Chinese population, but the conclusion is inconsistent. Objective To comprehensively evaluatethe association between uric acid level and vascular dementia in the Chinese population by meta-analysis. Methods The casecontrol studies on the relationship between uric acid level and vascular dementia in the Chinese population were searched inPubMed, Web of Science, Cochrane Library, CNKI, CBM, VIP and Wanfang Data by combining subject terms and free words,and the retrieval time limit was from the establishment of each database to July 2021. The case group was vascular dementiapatients, and the control group was non-vascular dementia patients. Data extraction and methodological quality of the includedliterature were independently completed by two researchers and any disagreements were resolved by the third researcher. RevMan5.2 software was used for meta-analysis. Stata 11.0 statistical software was used for sensitivity analysis and publication biasevaluation. Results Finally, 12 literature were included, involving 1 530 patients, including 890 cases in the case group and640 cases in the control group. Among the 12 literature included, the average age of patients in 2 literature was ≤ 65 years old,and the average age of patients in 10 literature was > 65 years old. The methodological quality evaluation results showed that theNewcastle-Ottawa Scale (NOS) scores of the included literature were all ≥ 6, indicating that the included literature were of highquality. The results of meta-analysis showed that there was no significant difference in uric acid level between the case group andthe control group [MD=10.66, 95%CI (-23.44, 44.76) , P=0.54] . The results of subgroup analysis showed that among patients≤65 years old, the uric acid level of the case group was higher than that of the control group [MD=144.60, 95%CI (16.71, 272.49) ,P=0.03] . The results of sensitivity analysis showed that there was no significant change in the amount of merger effect before andafter excluding each literature, suggesting that the results of this study were stable. The result of Begg test showed that P=0.602;the result of Egger test showed thatP=0.476; it indicated that there was no obvious publication bias in the included literature.Conclusion The level of uric acid in Chinese population may not be related to the occurrence of vascular dementia, but inpeople ≤ 65 years old, the level of uric acid may be related to the occurrence of vascular dementia.
