体外膜肺氧合联合急诊经皮冠状动脉介入术成功抢救急性心肌梗死并心脏停搏一例报道

摘要:

急性ST段抬高型心肌梗死(STEMI)患者如果出现心脏停搏等严重并发症,则会影响经皮冠状动脉介入术(PCI)的实施,而建立体外膜肺氧合(ECMO)可替代心脏泵血功能,保障急诊PCI安全有效。本文报道的国药同煤总医院心脏中心收治的1例溶栓后急性心肌梗死患者反复出现心脏停搏,在ECMO、呼吸机辅助下行急诊PCI,经主动脉内球囊反搏术及床旁连续性肾脏替代疗法等技术抢救成功后出院。随访2个月患者心脏、神经系统功能恢复良好。表明ECMO联合急诊PCI在急性心肌梗死并心脏停搏患者中的应用效果较好。

Abstract:

【Abstract】 Serious complications such as cardiac arrest will affect the implementation of percutaneous coronaryintervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI) . The establishment of extracorporealmembrane oxygenation (ECMO) can replace the function of pumping blood from the heart and ensure the safety and effectivenessof emergency PCI. In this paper, a case of acute myocardial infarction after thrombolysis with recurrent cardiac arrest treated in theHeart Center of Sinopharm Tongmei General Hospital was reported. After emergency PCI with ECMO and ventilator assistance,he was successfully rescued by intra-aortic balloon counterpulsation and bedside hemofiltration and discharged. After 2 months'follow-up, the function of heart and nervous system recovered well. It shows that ECMO combined with emergency PCI has abetter application effect in patients with acute myocardial infarction and cardiac arrest.
