背景 根据以往全球严重的新型肺炎暴发和病毒流行的心理状况影响的经验,制定和实施心理评估、支持、治疗和服务是新型冠状病毒肺炎(COVID-19)暴发卫生应对的关键和紧迫目标。护士是抗击 COVID-19 的医护人员的核心力量,尽管有学者发现抗击 COVID-19 的一线护士出现了心理问题,但无法有针对性地对其提供心理干预,因此亟待了解影响一线护士焦虑的相关因素。目的 了解武汉市首批收治 COVID-19 定点医院的临床一线护士的焦虑状况并分析其影响因素,为进一步做好心理护理提供依据。方法 2020 年 1—2 月选取湖北省武汉市首批定点收治COVID-19的三级医院(包括武汉市第五医院、武汉市第一医院)中在一线参与筛查治疗COVID-19的护士,共68人。应用自行编制的一般资料调查表及汉密尔顿焦虑量表(HAMA)对抗击 COVID-19 的一线护士进行调查,一线护士HAMA 得分影响因素分析采用多因素 Logistic 回归分析。结果 共计调查 68 人,其中 2 份问卷填写不完整,予以剔除,共回收有效问卷 66 份,有效回收率为 97.1%。66 名一线护士有 81.8%(54/66)出现焦虑;单因素分析显示,不同性别、年龄、工龄及参与抗击 COVID-19 临床工作时间一线护士 HAMA 得分比较,差异均有统计学意义(P<0.05);性别〔OR=11.435,95%CI(2.327,56.178)〕、工龄〔OR=2.196,95%CI(1.005,4.801)〕及参与抗击 COVID-19 临床工作时间〔OR=4.160,95%CI(1.508,11.476)〕是武汉市一线护士 HAMA 得分的独立影响因素(P<0.05)。结论 武汉市首批收治 COVID-19 定点医院的一线护士焦虑情绪较严重,有多种因素导致焦虑情况,应重视对一线护士心理支持和健康指导,提前做好参与救援护士的心理干预非常必要。

Abstract:

Background In response to the COVID-19 outbreak,one of the key urgent measures for addressing thenegative psychological influence is providing psychological assessment,support,and treatment services developed based on theexperience of coping with the negative psychosocial impact from previous severe new pneumonia outbreaks and viral epidemicsworldwide.Nurses are a core force in fighting against the COVID-19 epidemic.Some of them have been found with psychologicalproblems during the fighting,but no appropriate psychological interventions could be provided to them without investigation.So it is urgent to investigate the factors associated with anxiety in them.Objective To investigate the prevalence of anxietyand influencing factors among frontline nurses combating the COVID-19 epidemic in the first batch of designated hospitals forCOVID-19 treatment in Wuhan,providing data for further improving the psychological interventions.Methods This survey wasconducted from January to February 2020.Participants were the first group of nurses(n=68) participating in frontline screeningand treatment of COVID-19 recruited from the first batch of designated tertiary hospitals(including Wuhan Fifth Hospitaland Wuhan No.1 Hospital) for COVID-19 treatment in Wuhan,Hubei Province.A self-developed Demographic InformationQuestionnaire and the Hamilton Anxiety Scale(HAMA) were used to survey them.Multivariate Logistic regression analysiswas used to analyze the influencing factors of anxiety in them.Results Two cases were excluded due to a failure to complete thequestionnaires,and other 66(97.1%) were included for final analysis.The prevalence of anxiety was found to be 81.8%(54/66).Univariate analysis showed that the HAMA score significantly differed by gender,age,length of service and time for combatingthe COVID-19 epidemic(P<0.05).Gender 〔OR=11.435,95%CI(2.327,56.178)〕,length of service 〔OR=2.196,95%CI(1.005,4.801)〕 and time for combating the COVID-19 epidemic 〔OR=4.160,95%CI(1.508,11.476)〕 werethe indepedent influencing factors of HAMA score(P<0.05).Conclusion The prevalence of anxiety in this group of nurses ishigh,which is suggested to be caused by a variety of factors.Therefore,psychological support and health guidance for frontlinenurses should be attached more importance.And it is necessary to provide psychological intervention for them in advance.
