摘要：

目的　分析老年慢性阻塞性肺疾病（COPD）患者自我超越水平及其影响因素。方法　选取 2018 年 4—11 月武汉市中心医院和武汉大学中南医院呼吸内科收治的老年 COPD 患者 154 例，分别采用一般资料调查表、自我超越量表（STS）、Herth 希望指数量表（HHI）及领悟社会支持量表（PSSS）调查患者一般资料、自我超越水平、希望水平及领悟社会支持水平。HHI 及其各维度评分、PSSS 及其各维度评分与老年 COPD 患者 STS 评分的相关性分析采用 Pearson 相关分析，比较不同一般资料老年 COPD 患者 STS 评分，而老年 COPD 患者 STS 评分相关因素分析采用多元线性回归分析。结果　（1）本次调查共发放 154 份问卷调查，有效回收率为 93.51%（144/154）；本组 144 例老年COPD 患者 STS 评分为（40.17±7.49）分，其中 >45 分者 39 例（占 27.08%）。（2）Pearson 相关分析结果显示，HHI评分（r=0.396）、对现实与未来的积极态度评分（r=0.378）、采取的积极行动评分（r=0.412）、与他人保持亲密的关系评分（r=0.401）、PSSS 评分（r=0.479）、家庭内支持评分（r=0.501）及家庭外支持评分（r=0.458）与老年COPD 患者 STS 评分呈正相关（P<0.05）。（3）不同性别、婚姻状况、家庭人均月收入、肺功能分级及有无宗教信仰的老年 COPD 患者 STS 评分比较，差异无统计学意义（P>0.05）；不同年龄、文化程度、病程及有无合并其他类型慢性病的老年 COPD 患者 STS 评分比较，差异有统计学意义（P<0.05）。（4）多元线性回归分析结果显示，HHI 评分、PSSS 评分、文化程度及病程与老年 COPD 患者 STS 评分独立相关（P<0.05）；拟合的回归方程 Y=14.032+0.378X 1 （PSSS评分）-0.239X 2 （病程）+0.236X 3 （HHI 评分）+0.224X 4 （文化程度），差异有统计学意义（F=25.936，P<0.01；R 2 =0.427，调整 R 2 =0.411）。结论　老年 COPD 患者自我超越水平较低，而希望水平、领悟社会支持水平、文化程度、病程是老年 COPD 患者自我超越水平的影响因素。

Abstract：

Objective　To analyze the self-transcendence and its influencing factors in elderly patients with chronicobstructive pulmonary disease（COPD）. Methods　From April to November 2018，a total of 154 elderly patients withCOPD were selected in the Departments of Respiratory Medicine，the Central Hospital of Wuhan and Zhongnan Hospital ofWuhan University，general information，self-transcendence，hope and perceived social support was investigated by GeneralInformation Questionnaire，Self-transcendence Scale（STS），Herth Hope Index（HHI）and Perceived Social Support Scale（PSSS），respectively. Pearson correlation analysis was used to analyze the correlations of HHI and its each dimension score，PSSS and its each dimension score with STS score in elderly patients with COPD，and STS score was compared in patients withdifferent general information，moreover multiple linear regression analysis was used to analyze the related factors of STS score inelderly patients with COPD. Results　（1）The effective response rate was 93.51%（144/155），and the STS score of the 144elderly patients with COPD was（61.42±9.23），thereinto STS score was more than 45 in 39 cases（accounting for 27.08%）. （2）Pearson correlation analysis results showed that，HHI score （r=0.396），positive attitude towards reality and future score （r=0.378），positive taken actions score（r=0.412），maintain a close relationship with others score（r=0.401），PSSS score（r=0.479），internal family support score（r=0.501）and external family support score（r=0.458）was negatively correlated with STS scorein elderly patients with COPD，respectively（P<0.05）.（3）There was no statistically significant difference in STS score inpatients with different gender，marriage status，family monthly income per person or pulmonary function grade，as well as withreligious belief or not（P>0.05），while there was statistically significant difference in STS score in patients with different age，education level and course of disease，as well as with other types of chronic diseases or not，respectively（P<0.05）.（4）Multiple linear stepwise regression analysis results showed that，HHI score，PSSS score，education level and course of diseasewere independently related to the STS score in elderly patients with COPD（P<0.05），with statistically significant differencein the fitted regression equation（F=25.936，P<0.01；R 2 =0.427，adjusted R 2 =0.411），that was Y=14.032+0.378X 1 （PSSSscore）-0.239X 2 （course of disease）+0.236X 3 （HHI score）+0.224X 4 （education level）. Conclusion　Self-transcendenceis relatively low in elderly patients with COPD，hope，perceived social support，educational level and course of disease areinfluencing factors of self-transcendence in elderly patients with COPD.
