摘要:

 目的 探讨冠状动脉微血管疾病(CMVD)患者心室复极指标的变化。方法 选取2019年7月—2022 年3月陕西省人民医院收治的因心绞痛、心前区不适症状就诊的患者90例为研究对象,其中30例患者的冠状动脉CT血 管造影(CTA)或冠状动脉造影(CAG)检查显示心外膜冠状动脉狭窄50%~90%,将其纳入CAD组;30例患者的冠 状动脉CTA或CAG检查显示心外膜冠状动脉狭窄<50%,且单光子发射计算机断层成像术(SPECT)测定的冠状动脉 血流储备(CFR)<2.0,将其纳入CMVD组;30例患者的冠状动脉CTA或CAG检查显示心外膜冠状动脉狭窄<50%, 且SPECT的测定CFR≥2.0,将其纳入对照组。收集三组患者的一般资料、血脂指标,采用12导联心电图检查检测心室 复极指标〔QT间期、校正的QT间期(QTc)、QT间期离散度(QTd)、JT间期、校正的JT间期(JTc)、JT间期离散度 (JTd)〕。结果 CAD组低密度脂蛋白胆固醇(LDL-C)高于对照组和CMVD组(P<0.05)。CAD组和CMVD组QT间 期、QTc、QTd、JT间期、JTc长于对照组,CMVD组JTd长于CAD组和对照组(P<0.05)。结论 CMVD患者心电图表 现为QT间期、QTc、QTd、JT间期、JTc及JTd延长。

英文摘要:

Objective To investigate the changes in ventricular repolarization indexes in patients with coronary microvascular disease (CMVD) . Methods A total of 90 patients with angina pectoris and precordial discomfort symptoms admitted to Shaanxi Provincial People's Hospital from July 2019 to March 2022 were selected as the study subjects. Among them, 30 patients, coronary artery CT angiography (CTA) or coronary angiography (CAG) examination showed that the epicardial coronary artery stenosis was 50% to 90%, and they were included in the CAD group; 30 patients, coronary artery CTA or CAG examination showed that the epicardial coronary artery stenosis was < 50%, and single-photon emission computed tomography (SPECT) showed that the coronary flow reserve (CFR) was < 2.0, and they were included in the CMVD group; 30 patients' coronary artery CTA or CAG examination showed that the epicardial coronary artery stenosis was < 50 %, and SPECT showed that the CFR was ≥ 2.0, and they were included in the control group. The general data and blood lipid indexes of patients in the three groups were collected, and the ventricular repolarization indexes [QT interval, QT interval correction (QTc) , QT interval dispersion (QTd) , JT interval, JT interval correction (JTc) , JT interval dispersion (JTd) ] were detected by 12-lead electrocardiography. Results The low density lipoprotein cholesterol (LDL-C) in the CAD group was higher than that in the control group and CMVD group (P < 0.05) . The QT interval, QTc, QTd, JT interval, and JTc in the CAD group and CMVD group were longer than those in the control group, and JTd in the CMVD group was longer than that in the CAD group and control group (P < 0.05) . Conclusion The electrocardiogram of CMVD patients shows prolongation of QT interval, QTc, QTd, JT interval, JTc, and JTd.
