磁共振成像与双层光谱探测器 CT 在急性缺血性 脑卒中患者脑血流灌注评估中的应用效果

摘要:

 目的 探讨磁共振成像(MRI)与双层光谱探测器CT在急性缺血性脑卒中患者脑血流灌注评估中 的应用效果。方法 选取2021年1月—2022年1月开滦总医院接诊的100例急性缺血性脑卒中患者为研究对象,所有 患者进行MRI检查与双层光谱探测器CT检查。记录患者MRI检查结果{患侧与健侧脑血流灌注指标〔平均通过时间 (MTT)、达峰时间(TTP)、脑血流量(CBF)、脑血容量(CBV)〕及病灶面积}和双层光谱探测器CT检查结 果〔患侧与健侧脑血流灌注指标(碘密度、有效原子序数、CBF、CBV)及病灶面积〕。结果 MRI检查结果显示, 急性缺血性脑卒中患者患侧MTT、TTP长于健侧,CBF、CBV小于健侧(P<0.05);双层光谱探测器CT检查结果显 示,急性缺血性脑卒中患者患侧碘密度、有效原子序数低于健侧,CBF、CBV小于健侧(P<0.05);双层光谱探测器 CT检查的急性缺血性脑卒中患者患侧CBF、CBV小于MRI(P<0.05)。结论 针对急性缺血性脑卒中患者,与MRI相 比,采用双层光谱探测器CT检查的CBF、CBV较小,脑血流灌注较差。

Abstract:

Objective To explore the application effect of magnetic resonance imaging (MRI) and double-layer spectral detector CT in the evaluation of cerebral blood perfusion in patients with acute ischemic stroke. Methods A total of 100 patients with acute ischemic stroke admitted to Kailuan General Hospital from January 2021 to January 2022 were selected as the research objects. All patients underwent MRI examination and double-layer spectral detector CT examination. The results of MRI examination {cerebral blood flow perfusion indexes of affected side and healthy side [mean transit time (MTT) , time to peak (TTP) , cerebral blood flow (CBF) , cerebral blood volume (CBV) ] and lesion area} and the results of double-layer spectral detector CT examination [cerebral blood flow perfusion indexes of affected side and healthy side (iodine density, effective atomic number, CBF, CBV) and lesion area] were recorded. Results The results of MRI examination showed that the MTT and TTP of the affected side were longer than those of the healthy side, and CBF and CBV were smaller than those of the healthy side in patients with acute ischemic stroke (P < 0.05) . The results of double-layer spectral detector CT examination showed that the iodine density and effective atomic number of the affected side were lower than those of the healthy side in patients with acute ischemic stroke, and CBF and CBV were smaller than those of the healthy side in patients with acute ischemic stroke (P < 0.05) . In patients with acute ischemic stroke, CBF and CBV of the affected side detected by double-layer spectral detector CT were smaller than those detected by MRI (P < 0.05) . Conclusion For patients with acute ischemic stroke, compared with MRI, the CBF and CBV detected by double-layer spectral detector CT are smaller and the cerebral blood perfusion is worse.
