摘要:
目的 探讨连接蛋白2(JP2)和成纤维细胞生长因子23(FGF23)与心房颤动合并心力衰竭的关系。方法 选取2020—2022年于河北省人民医院住院的心房颤动患者44例为心房颤动组,心房颤动合并心力衰竭患者65例为心房颤动合并心力衰竭组。比较两组临床资料,采用单因素及多因素Logistic回归分析探讨心房颤动合并心力衰竭的影响因素。结果 两组心率、有冠心病史者占比、有高血压史者占比、肌酐、尿素氮、肾小球滤过率(GFR)、尿酸、JP2、N末端脑钠肽前体(NT-proBNP)、左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)、右心室内径、右心房内径比较,差异有统计学意义(P<0.05)。单因素Logistic回归分析结果显示,JP2、NT-proBNP、LVEDD、LVESD、LVEF、右心室内径、右心房内径可能是心房颤动合并心力衰竭的影响因素(P<0.05)。多因素Logistic回归分析结果显示,JP2〔OR=0.947,95%CI(0.910,0.986)〕、NT-proBNP〔OR=1.001,95%CI(1.000,1.001)〕是心房颤动合并心力衰竭的独立影响因素(P<0.05)。结论 JP2是心房颤动合并心力衰竭的独立影响因素;未发现FGF23与心房颤动合并心力衰竭相关。
Abstract:
Objective To investigate the relationship between junctophilin-2 (JP2) and fibroblast growth factor 23(FGF23) and atrial fibrillation complicated with heart failure. Methods A total of 44 patients with atrial fibrillation admittedto Hebei General Hospital from 2020 to 2022 were selected as the atrial fibrillation group, 65 patients with atrial fibrillationcomplicated with heart failure were selected as the atrial fibrillation complicated with heart failure group. Clinical data werecompared between the two groups. Univariate and multivariate Logistic regression analysis were used to analyze the influencingfactors of atrial fibrillation complicated with heart failure. Results There were significant differences in the heart rate, proportionof history of coronary heart disease, proportion of history of hypertension, creatinine, urea nitrogen, glomerular filtration rate (GFR) ,uric acid, JP2, N-terminal pro-brain natriuretic peptide (NT-proBNP) , left ventricular end-diastolic diameter (LVEDD) , leftventricular end-systolic diameter (LVESD) , left ventricular ejection fraction (LVEF) , right ventricular diameter, and right atrialdiameter between the two groups (P < 0.05) . Univariate Logistic regression analysis showed that the JP2, NT-proBNP, LVEDD,LVESD, LVEF, right ventricular diameter, and right atrial diameter may be the influencing factors of atrial fibrillation complicatedwith heart failure (P < 0.05) . Multivariate Logistic regression analysis showed that the JP2 [OR=0.947, 95%CI (0.910, 0.986) ] ,NT-proBNP [OR=1.001, 95%CI (1.000, 1.001) ] were the influencing factors of atrial fibrillation complicated with heart failure (P< 0.05) . Conclusion JP2 is the influencing factor of atrial fibrillation complicated with heart failure, no association was foundbetween FGF23 and atrial fibrillation combined with heart failure.
