摘要:

 目的 分析脑血管周围间隙扩大(EPVS)与肱踝脉搏波传导速度(baPWV)的关系。方法 回顾性选取2020年6—12月在北京市中关村医院进行baPWV检测及颅脑MRI检查的成年体检者201例为研究对象。收集受试者一般资料、baPWV、颅脑MRI检查结果。根据baPWV,将所有受试者分为baPWV正常组(baPWV<14 m/s,56例)和baPWV增快组(baPWV≥14 m/s,145例)。根据EPVS发生情况,将所有受试者分为EPVS组50例和非EPVS组151例。采用多因素Logistic回归分析探讨成年体检者发生EPVS的影响因素。结果 baPWV增快组年龄大于baPWV正常组,吸烟者占比、高血压发生率、糖尿病发生率、收缩压、BMI、高密度脂蛋白胆固醇(HDL-C)、总胆固醇(TC)、EPVS发生率高于baPWV正常组(P<0.05)。EPVS组年龄大于非EPVS组,高血压发生率、收缩压、舒张压高于非EPVS组,baPWV快于非EPVS组(P<0.05)。多因素Logistic回归分析结果显示,年龄〔OR=1.121,95%CI(1.056,1.190)〕、收缩压〔OR=1.129,95%CI(1.074,1.187)〕、baPWV〔OR=1.296,95%CI(1.060,1.583)〕是成年体检者发生EPVS的独立影响因素(P<0.05)。结论 baPWV增快是成年人群发生EPVS的危险因素。

Abstract:

stract】 Objective To investigate the relationship between enlarged perivascular spaces (EPVS) and brachialankle pulse wave velocity (baPWV) . Methods A total of 201 cases of adults who underwent baPWV detection and brain MRIexamination in Beijing Zhongguancun Hospital from June to December 2020 were retrospectively selected as the study objects.General data, baPWV and brain MRI examination results were collected. According to baPWV, all subjects were divided intonormal baPWV group (baPWV < 14 m/s, 56 cases) and increased baPWV group (baPWV ≥ 14 m/s, 145 cases) . According tothe occurrence of EPVS, all subjects were divided into EPVS group (50 cases) and non-EPVS group (151 cases) . MultivariateLogistic regression analysis was used to explore the influencing factors of EPVS in adults undergoing physical examination.Results The age of increased baPWV group was older than that of normal baPWV group, and the proportion of smokers, theincidence of hypertension, the incidence of diabetes, systolic blood pressure, BMI, high density lipoprotein cholesterol (HDL-C) ,total cholesterol (TC) , and the incidence of EPVS were higher than those of normal baPWV group (P < 0.05) . The age of EPVSgroup was older than that of non-EPVS group, the incidence of hypertension, systolic blood pressure and diastolic blood pressurewere higher than those of non-EPVS group, and baPWV was faster than that of non-EPVS group (P < 0.05) . Multivariate Logisticregression analysis showed that age [OR=1.121, 95%CI (1.056, 1.190) ] , systolic blood pressure [OR=1.129, 95%CI (1.074,1.187) ] , baPWV [OR=1.296, 95%CI (1.060, 1.583) ] were the independent influencing factors of EPVS in adult physicalexamination subjects (P < 0.05) . Conclusion The increase of baPWV is a risk factor for EPVS in the adult population.
