基于微型营养评分法的干预措施对老年下肢骨折患者营养状况的影响研究

摘要:

目的 探讨基于微型营养评分法(MNA)的干预措施对老年下肢骨折患者营养状况的影响。方法 回顾性收集2019年4月至2021年4月河北工程大学附属医院收治的接受基于MNA的干预措施的老年下肢骨折患者43例为观察组,依据1∶1原则选择同期于本院接受常规护理方案的老年下肢骨折患者43例为对照组。对照组给予常规治疗及护理方案,观察组在对照组基础上给予基于MNA的干预措施。比较两组患者入院时和出院时的营养指标(前白蛋白、血红蛋白、转铁蛋白、白蛋白水平)、术后切口愈合情况(切口愈合时间、切口干燥时间及切口皮肤坏死情况)。结果入院时两组患者前白蛋白、血红蛋白、转铁蛋白、白蛋白水平比较,差异无统计学意义(P>0.05);出院时两组患者前白蛋白、血红蛋白、转铁蛋白、白蛋白水平分别高于本组入院时,且观察组高于对照组(P<0.05)。观察组患者术后切口愈合时间、切口干燥时间短于对照组,术后切口皮肤坏死所占比例低于对照组(P<0.05)。结论 基于MNA的干预措施有利于老年下肢骨折患者营养状况的改善及切口的愈合。

Abstract:

【Abstract】 Objective To explore the effect of intervention measures based on Mini-Nutrition Assessment (MNA)on the nutritional status of elderly patients with lower extremity fractures. Methods Forty-three elderly patients with lowerextremity fractures who received intervention measures based on MNA in Affiliated Hospital of Hebei Engineering University fromApril 2019 to April 2021 were retrospectively collected as the observation group, and 43 elderly patients with lower extremityfractures who received routine nursing program in the same hospital during the same period were selected as the control groupaccording to the principle of 1∶1. The control group was given routine treatment and nursing programs, and the observation groupwas given MNA-based intervention measures on the basis of the control group. The nutritional indicators (levels of prealbumin,hemoglobin, transferrin, albumin) and postoperative wound healing (incision healing time, incision drying time, proportion ofincision skin necrosis) were compared between the two groups of patients at admission and discharge. Results There was nosignificant difference in the levels of prealbumin, hemoglobin, transferrin and albumin between the two groups at admission (P> 0.05) . The levels of prealbumin, hemoglobin, transferrin and albumin in the two groups at discharge were higher than those atadmission, respectively, and the levels of prealbumin, hemoglobin, transferrin and albumin in the observation group were higherthan those in the control group (P < 0.05) . The postoperative incision healing time and incision drying time in the observationgroup were shorter than those in the control group, and the proportion of postoperative incision skin necrosis was lower than thatin the control group (P < 0.05) .Conclusion Intervention measures based on MNA are beneficial to improving nutritional statusand promoting wound healing in elderly patients with lower extremity fractures.
