摘要：

目的 比较CleanerTM旋转式血栓清除系统、AngioJet血栓清除系统及Straub血栓清除系统在下肢深静脉血栓形成(LEDVT)中的应用效果及安全性。方法 在医院病例管理系统中选取2019年6月至2021年11月中国人民解放军空军军医大学西京医院甲乳血管外科收治的LEDVT患者64例,其中经CleanerTM旋转式血栓清除系统治疗者14例(A组)、经AngioJet血栓清除系统治疗者30例(B组)、经Straub血栓清除系统治疗者20例(C组)。比较三组患者临床资料、术后即刻血栓清除效果、并发症发生率及血栓复发率;LEDVT患者术后即刻血栓清除效果影响因素分析采用多因素Logistic回归分析。结果 三组患者有肿瘤史者占比、有外科手术史者占比及术前血浆纤维蛋白原比较,差异有统计学意义(P<0.05)。三组患者术后即刻血栓清除效果、并发症发生率及血栓复发率比较,差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,腔内血栓清除术类型并不是LEDVT患者术后即刻血栓清除效果的独立影响因素〔OR=1.333,95%CI(0.124,14.309),P>0.05〕。结论 CleanerTM旋转式血栓清除系统在LEDVT中的应用效果不劣于AngioJet血栓清除系统和Straub血栓清除系统,且安全性较高。

Abstract：

【Abstract】 Objective To compare the clinical effect and safety of CleanerTM rotational thrombectomy system, AngioJetthrombectomy system and Straub thrombectomy system in the treatment of lower extremity deep-venous thrombosis (LEDVT) .MethodsFrom the Hospital Case Management System, 64 patients with LEDVT in the Department of Thyroid, Breast andVascular Surgery, Xijing Hospital, the Air Force Medical University from June 2019 to November 2021 were selected, including14 patients treated by CleanerTM rotational thrombectomy system (group A) , 30 patients treated by AngioJet thrombectomy system(group B) and 20 patients treated by Straub thrombectomy system (group C) . The clinical data, effect of immediate thrombectomyafter operation, incidence of complications and the recurrence rate of thrombus were compared among the three groups;multivariate Logistic regression analysis was used to analyze the influencing factors of effect of immediate thrombectomy afteroperation in LEDVT patients. Results There were significant differences in the proportion of patients with tumor history, surgeryhistory and preoperative plasma fibrinogen among the three groups (P <0.05) . There was no significant difference in the effectof immediate thrombectomy after operation, incidence of complications and recurrence rate of thrombus among the three groups(P>0.05) . Multivariate Logistic regression analysis showed that the type of endovascular thrombectomy was not an independentfactor of effect of immediate thrombectomy after operation in LEDVT patients [OR=1.333, 95%CI (0.124, 14.309) , P > 0.05] .Conclusion The application effect of CleanerTM rotary thrombectomy system in LEDVT is not inferior to Angiojet thrombectomysystem and Straub thrombectomy system, and the safety is high.
