药物与临床硫酸镁联合硝苯地平治疗妊娠高血压的效果研究

摘要:
背景妊娠高血压是一类危害极大的妊娠并发症,临床中对于该病的治疗尚处于探索阶段,未形成统一的治疗标准,而硫酸镁联合硝苯地平治疗该病的疗效仍缺少试验和理论支持。目的 探讨硫酸镁联合硝苯地平治疗妊娠高血压的治疗效果,为临床用药提供理论依据。方法 选取2018年10月至2020年10月在合肥市第八人民医院接受治疗的妊娠高血压患者216例为研究对象,应用随机数字表法分为对照组与观察组,各108例。对照组给予硫酸镁治疗,观察组给予硫酸镁联合硝苯地平治疗。比较两组患者临床疗效,治疗前后收缩压、舒张压和24 h尿蛋白定量,治疗前和分娩后健康调查简表(SF-36)评分〔生理机能(PF)、生理职能(RP)、躯体疼痛(BP)、精力(VT)、社会功能(SF)、情感职能(RE)、精神健康(MH)、一般健康状况(GH)评分〕,不良妊娠结局发生率、不良反应发生率。结果 观察组患者临床疗效优于对照组(P<0.05)。治疗后对照组、观察组患者收缩压、舒张压、24 h尿蛋白定量分别低于本组治疗前,且观察组患者低于对照组(P<0.05)。分娩后对照组、观察组患者PF、RP、BP、VT、SF、RE、MH、GH评分分别高于本组治疗前,且观察组患者高于对照组(P<0.05)。观察组不良妊娠结局发生率为6.5%,低于对照组的23.2%(χ2=11.886,P=0.001);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论硫酸镁联合硝苯地平能有效提高妊娠高血压患者的临床疗效,降低血压及不良妊娠结局发生率,并提升患者生活质量,值得临床借鉴与推广。
Abstract:
【Abstract】 Background Gestational hypertension is a kind of harmful pregnancy complications, and clinicaltreatment is still in the exploration stage. It does not form a unified treatment standard. Magnesium sulfate combined withnifedipine in the treatment of gestational hypertension is still lack of experimental and theoretical support. Objective To analyzethe effect of magnesium sulfate combined with nifedipine in the treatment of gestational hypertension and provide theoretical basisfor clinical medication. Methods A total of 216 patients with gestational hypertension treated in Hefei Eighth People's Hospitalfrom October 2018 to October 2020 were selected as subjects. They were divided into control group and observation group byrandom number table method and each group had 108 patients. Control group was treated with magnesium sulfate and observationgroup was treated with magnesium sulfate combined with nifedipine. Clinical efficacy, systolic pressure, diastolic pressure and 24 hurinary protein quantification before and after treatment, Short Form-36 (SF-36) [physical functioning (PF) , role-physical (RP) ,bodily pain (BP) , vitality (VT) , social functioning (SF) , role-emotional (RE) , mental health (MH) , general health (GH) ] scorebefore treatment and after delivery, incidence of adverse pregnancy outcome, and incidence of adverse reactions were comparedbetween two groups. Results The clinical efficacy of the observation group was better than of the control group (P < 0.05) . Aftertreatment, diastolic pressure, systolic pressure and 24 h urinary protein quantification of the control group and the observationgroups were lower than those before treatment respectively, and diastolic pressure, systolic pressure and 24 h urinary proteinquantification of the observation group were lower than those of the control group (P < 0.05) . After delivery, PF, RP, BP, VT,SF, RE, MH, GH scores of the control group and the observation group were higher than those before treatment respectively, andPF, RP, BP, VT, SF, RE, MH, GH scores of observation group were higher than those of control group (P < 0.05) . The incidenceof adverse pregnancy outcomes of the observation group (6.5%) was lower than that of the control group (23.2%) (χ2 =11.886,P=0.001) . There was no significant difference in the incidence of adverse reactions between the two groups (P > 0.05) .ConclusionMagnesium sulfate combined with nifedipine can improve treatment effect of patient with gestational hypertension,lower blood pressure, reduce incidence of adverse pregnancy outcomes and improve the quality of life of patients. This treatmentis worth reference and promotion.
