摘要:

 背景 抑郁症在高血压患者中非常普遍,其已被证实与低度全身炎症有关。系统性免疫炎症指数 (SII)是一种综合性的新型炎症指标,是评估全身炎症和免疫应答之间平衡状态的客观指标,与多种疾病相关。然 而,SII与高血压患者发生抑郁症的相关性尚不明确。目的 探索SII与高血压患者发生抑郁症的关系。方法 选取2020 年7月至2021年4月在三峡大学附属仁和医院心血管内科住院治疗的高血压患者270例为研究对象。收集患者入院时的 一般资料、实验室检查指标。根据汉密尔顿抑郁量表(HAMD)评分将患者分为无抑郁症组(HAMD评分<20分,240 例)和抑郁症组(HAMD评分≥20分,30例)。根据SII四分位数将患者分为Q1组(SII≤330.60,72例)、Q2组(SII 为330.61~426.43,68例)、Q3组(SII为426.44~695.64,68例)、Q4组(SII>695.64,62例)。采用多因素Logistic回 归分析探讨高血压患者发生抑郁症的影响因素。结果 抑郁症组合并冠心病者所占比例、SII高于无抑郁症组,服药种 数多于无抑郁症组(P<0.05)。多因素Logistic回归分析结果显示,SII>695.64是高血压患者发生抑郁症的影响因素 〔OR=4.803,95%CI(1.443,15.980),P<0.05〕。高血压患者抑郁症发生率随着SII的升高而升高(P<0.05)。结 论 SII>695.64是高血压患者发生抑郁症的危险因素,且抑郁症发生风险随SII的升高而升高。

Abstract:

 Background Depression is very common in patients with hypertension and has been shown to be associated with low-grade systemic inflammation. The systemic immune-inflammation index (SII) is a comprehensive novel inflammatory index, an objective marker to assess the balance between systemic inflammation and immune response, and is associated with a variety of diseases. However, the relationship between SII and depression in patients with hypertension is not clear. Objective To explore the relationship between SII and depression in patients with hypertension. Methods A total of 270 patients with hypertension hospitalized in the Internal Medicine-Cardiovascular Department of Affiliated Renhe Hospital of China Three Gorges University from July 2020 to April 2021 were selected as the research objects. The general data and laboratory examination indexes of patients were collected. According to the Hamilton Depression Scale (HAMD) score, the patients were divided into non-depression group (HAMD score 695.64, 62 cases) . Multivariate Logistic regression analysis was used to explore the influencing factors of depression in patients with hypertension. Results The proportion of patients with coronary heart disease and SII in the depression group were higher than those in the non-depression group, and the number of drugs taken was more than that in the non-depression group (P 695.64 was a risk factor of depression in hypertensive patients [OR=4.803, 95%CI (1.443, 15.980) , P 695.64 is a risk factor for depression in patients with hypertension, and the risk of depression increases with the increase of SII.
