中医·中西医结合参草通脉复方对慢性心力衰竭大鼠心肾功能的影响研究

摘要:
背景慢性心力衰竭(CHF)是多种疾病的终末期临床综合征,治疗与干预方式有限,因而探讨一种有效、安全的药物具有重要的临床意义。目的 探讨参草通脉复方对CHF大鼠心肾功能的影响,以期为该药的临床应用提供参考。方法 2019年9月至2020年1月,选取60只SPF级12周龄雄性大鼠,采用随机数字表法将其分为空白对照组、假手术组、模型组、西药组、中药1组、中药2组、中药3组,每组8只。除空白对照组、假手术组外,其他组大鼠均制备CHF模型,模型制备成功后空白对照组、假手术组、模型组大鼠仅常规予以等容积的蒸馏水进行灌胃,西药组大鼠采用福辛普利钠片治疗,中药1组大鼠采用左归饮治疗,中药2组大鼠采用人参养荣汤治疗,中药3组大鼠采用参草通脉复方治疗,均持续干预28 d。比较七组大鼠干预前心功能指标〔心率、左心室射血分数(LVEF)、左心室收缩末期容积(LVESV)、左心室舒张末期容积(LVEDV)〕、肾功能指标〔血尿素氮(BUN)、血肌酐(Scr)〕及干预前后心、肾功能指标的差值。结果 七组大鼠干预前心率、LVEF、LVESV、LVEDV及BUN、Scr水平比较,差异有统计学意义(P <0.05)。中药3组大鼠干预前后心率的差值小于假手术组、模型组、西药组、中药1组、中药2组;干预前后LVEF的差值小于空白对照组,但大于假手术组、模型组、中药1组、中药2组;干预前后LVESV的差值小于假手术组、模型组、中药1组、中药2组;干预前后LVEDV的差值大于空白对照组,但小于假手术组、模型组、西药组、中药1组、中药2组;干预前后BUN水平的差值大于空白对照组、假手术组,但小于模型组、西药组、中药1组、中药2组;干预前后Scr水平的差值大于空白对照组,但小于模型组、西药组、中药1组、中药2组(P <0.05)。结论 与福辛普利钠片、左归饮、人参养荣汤相比,参草通脉复方可更有效地改善CHF大鼠的心肾功能。
Abstract:
【Abstract】 Background Chronic heart failure (CHF) is an end-stage clinical syndrome of multiple diseases with limitedtreatment and intervention methods, and it is of great clinical significance to explore an effective and safe drugs. Objective Toinvestigate the effect of Shencao Tongmai compound prescription on cardiac and renal functions in CHF rats, in order to providea reference for the clinical application of this drug. Methods From September 2019 to January 2020, 60 SPF 12-week-oldmale rats were selected and divided into blank control group, sham group, model group, western medicine group, group A, groupB, and group C by random number table method, with 8 rats in each group. Except rats in blank control group and sham group,CHF models were made in the other groups. After the successful preparation of the model, rats in the blank control group, shamgroup, model group were only intragastric administration with distilled water in equal volume, rats in the western medicine groupwere treated with fosinopril sodium tablets, rats in the group A were treated with Zuogui decoction, rats in the group B were treatedwith Renshen Yangrong decoction, rats in the group C were treated with Shencao Tongmai compound prescription. All rates wereintervented for 28 days. Cardiac function indexes [heart rate,left ventricular ejection fraction (LVEF) , left ventricular endsystolic volume (LVESV) , left ventricular end diastolic volume (LVEDV) ] and renal function indexes [blood urea nitrogen (BUN) ,serum creatinine (Scr) ] before intervention, and differential value of cardiac function and renal function indexes before and afterintervention were compared among the seven groups. Results There were significant differences in heart rate, LVEF, LVESV,LVEDV and levels of BUN, Scr before intervention among the seven groups (P < 0.05) . Differential value of heart rate before andafter intervention of group C was less than that of sham group, model group, western medicine group, group A, group B (P < 0.05) ;differential value of LVEF before and after intervention of group C was less than that of blank control group, and larger than that ofsham group, model group, group A, group B (P < 0.05) ; differential value of LVESV before and after intervention of group C wasless than that of sham group, model group, group A, group B (P < 0.05) ; differential value of LVEDV before and after interventionof group C was larger than that of blank control group, and less than that of sham group, model group, western medicine group,group A, group B (P < 0.05) ; differential value of BUN level of group C was larger than that of blank control group, sham group, andless than that of model group, western medicine group, group A, group B (P < 0.05) ; differential value of Scr level of group C waslarger than that of blank control group, and less than that of model group, western medicine group, group A, group B (P < 0.05) .Conclusion Compared with fosinopril sodium tablets, Zuogui decoction and Renshen Yangrong decoction, Shencao Tongmaicompound prescription can more effectively improve the cardiac and renal function of CHF rats.
